When Cheng Wei Jie and Cheng Wei Xiu couldn't reach their eldest brother, Cheng Wei Liang, Xiao Yan stepped in and falsely claimed that she had called the police, leading to his arrest for assaulting their father, Cheng Guo Dong. She told them that since their eldest brother was gone, they would have to take care of their father. Cheng Wei Jie and Cheng Wei Xiu angrily accused Xiao Yan of meddling in their family affairs and being heartless.
They tried to shirk responsibility, with Cheng Wei Xiu needing to cook for her child, Nuo Nuo, and Cheng Wei Jie having work at his store, blaming Xiao Yan for the predicament. Bai Shu, overhearing, criticized Xiao Yan, suggesting she had been abroad too long and had forgotten local customs.
He believed it was easy to preach morality when uninvolved, but daily care for an Alzheimer's patient was incredibly difficult, and he questioned who would now care for Cheng Guo Dong. Xiao Yan retorted that those who called others stupid were the most foolish, refusing to explain her actions. Cheng Guo Dong, suffering from Alzheimer's, became restless, constantly moving his oxygen mask, asking to close non-existent windows, and demanding water.
His children, Cheng Wei Jie and Cheng Wei Xiu, ignored his pleas, preoccupied with their phones and calls, secretly trying to avoid caretaking. When Cheng Guo Dong suffered an episode of bowel incontinence, both Cheng Wei Jie and Cheng Wei Xiu made excuses to leave, each trying to make the other clean up.
They eventually abandoned their father, telling the hospital staff that a caregiver they had hired would arrive soon and that if there were any issues, they should contact Cheng Wei Liang once he was out. Jiang Shan and a care worker then helped clean and change Cheng Guo Dong. Bai Shu remained critical of Xiao Yan, believing she caused these problems, but Xiao Yan still offered no explanation.
Later, Cheng Wei Liang returned to the hospital, much to his father's joy and relief. Cheng Guo Dong cried tears of happiness upon seeing his eldest son. Cheng Wei Liang revealed that Xiao Yan hadn't reported him to the police; instead, she had sent him home to rest, realizing his emotional distress. He explained that her actions allowed him to reflect on his past naughtiness and his father's boundless patience, leading him to understand his own mistakes.
He thanked Xiao Yan, saying she helped him realize what was truly important. Bai Shu, who was nearby, overheard this conversation and finally understood Xiao Yan's thoughtful intentions, realizing he had misjudged her. He felt a moment of awkwardness. Meanwhile, Jiang Shan, upon finishing her shift, rushed home frantically because she couldn't reach her husband, Xian Yun, by phone and worried he might be in danger.
Tang Hua, noticing Jiang Shan's haste, learned from another colleague, Lin Zhi, that Jiang Shan's husband had been paralyzed from the waist down in a car accident before their marriage eight or nine years ago. Despite her family's advice, Jiang Shan married him and has been devotedly caring for him ever since, managing both her demanding job in the Emergency Department and her home life.
Coincidentally, Lu Ping’an saw Jiang Shan struggling to find a taxi and offered her a ride home. Lu Ping'an, concerned, accompanied Jiang Shan up to her apartment. They found Xian Yun lying on the floor. He had tried to boil water for Jiang Shan, but his severe Central Pain Syndrome caused his hands to tremble, leading him to scald his leg and suffer urinary incontinence.
Lu Ping'an immediately helped Xian Yun into his wheelchair, bandaged his leg, and changed his soiled clothes, getting his own clothes wet in the process. Jiang Shan offered Xian Yun’s clean clothes, but Lu Ping'an politely declined, stating he had spare clothes in his car. After ensuring Xian Yun was settled, Lu Ping'an departed.
On his way out, he telephoned Director Song, asking for help to find an honest and capable male caregiver for Xian Yun, explaining that it wasn't convenient for him to arrange it directly through his own hospital. Back inside, Xian Yun expressed his shame to Jiang Shan for soiling Lu Ping'an's clothes and his feelings of being a burden and useless.
He yearned for them to have a child to complete their home, despite their prior agreement not to discuss it. Jiang Shan comforted him, assuring him that she was happy as long as he was alive and that he was already doing very well, making their home complete. At the Tongshan Hospital Emergency Department, He Chun Sheng was admitted after collapsing on a plane due to an unknown infection in his right leg, which was rotting and smelled foul.
Xu Yiran and Bai Shu observed extensive infection and decay of the patient's leg tissues, along with a persistent high fever and a risk of myocarditis. He Chun Sheng's wife informed them that he had recently hiked Yunling Mountain and saw snakes. His friend confirmed that He Chun Sheng was bitten by a cobra and treated with a local folk method since no antivenom was available.
Believing it was a cobra bite, Bai Shu ordered antivenom, but He Chun Sheng's condition worsened, indicating an allergic reaction. This led Bai Shu to conclude that snake venom was not the primary cause of the worsening infection. Xiao Yan then shared her experience with a similar patient abroad who had been exposed to rare wild monkey feces containing Group A Streptococcus, leading to Necrotizing Fasciitis, also known as flesh-eating disease.
She theorized that the snake bite created an open wound for the bacteria to enter. After the diagnosis was confirmed by the Municipal Center for Disease Prevention and Control, Bai Shu and Xu Yiran explained to He Chun Sheng's wife that without immediate amputation, the rapidly spreading Group A Streptococcus would cause widespread tissue necrosis and eventually death. With a heavy heart, his wife signed the consent form for the surgery to save his life.
Later, a high school student named Xiao Nan came to the Emergency Department complaining of dizziness. His mother, however, was convinced he was feigning illness to skip school and refused Xiao Yan's recommendation for a head CT scan, citing concerns about brain health during college entrance exam preparations and claiming he often faked illness. Xiao Yan, sensing something was wrong, tried to follow them after they left, but they had already disappeared.
Soon after, an emergency call for a chemical plant explosion victim with 70% body surface burns diverted her attention. The next day, Xiao Nan collapsed on his way to school. His mother rushed him back to the Tongshan Hospital Emergency Department. Xiao Nan arrived with no heartbeat or breathing, but Xiao Yan and the medical team successfully resuscitated him. A subsequent head CT scan revealed an acute brain hemorrhage in the posterior fossa, a highly dangerous and continuously bleeding condition.
Xiao Nan's mother, filled with remorse, pleaded with Xiao Yan to save her son, regretting her earlier refusal of the CT scan. During a case discussion, Lu Ping'an and other doctors were at a loss due to the hemorrhage's precarious location. Xiao Yan, however, made the bold decision to perform a craniotomy on Xiao Nan. Though a lateral ventricular drain successfully lowered intracranial pressure, the posterior fossa continued to bleed.
Despite the grim prognosis and the surgical team's concerns about the inoperable nature of such a hemorrhage, Xiao Yan refused to give up, determined to fight for the young patient's life.
