Chen Yan approached Bai Shu to warn him, but Bai Shu remained confident in his ability to perform the surgery. Jiang Zhongjing, after carefully reviewing Bai Shu's surgical plan, sternly warned him to devote two hundred percent of his effort to the operation, reminding him of severe consequences if anything went wrong.
Bai Shu affirmed his commitment. During Chen Chuan Shu's surgery, despite severe tissue adhesions that made the procedure more difficult than anticipated, Bai Shu remained calm and skillfully removed the heart tumor, successfully completing the operation. In a flashback, Lin Zhiyuan proposed to her without a ring, imagining their peaceful life in his hometown, Smoke Alley, after their mission. Heartbroken by his loss, Xiao Yan met with Director Katherine to tender her resignation, explaining her decision to leave Switzerland and possibly not return, not for another field mission, but for a new path.
The next morning, Bai Shu, on his way to an internal investigation regarding patient complaints, stopped for his usual handground coffee, only to be told it was the last cup from that particular batch. At the same coffee shop, Xiao Yan, rushing to a client meeting, accidentally bumped into him, spilling his coffee. She offered her own untouched coffee as compensation and quickly left. Annoyed, Bai Shu dismissed her offer, claiming she wouldn't understand the difference in coffee beans.
Noticing the medical equipment product introduction pamphlets in her hand, he mistakenly concluded she was a salesperson and accused her of being solely motivated by money. As Bai Shu emerged from the shop, he found a parking ticket on his car, while Xiao Yan rode away on her motorcycle. Shortly after, Bai Shu saw a crowd gathered around a pregnant woman, Wang Qing, by the roadside, where Xiao Yan was already performing CPR.
Bai Shu intervened, diagnosing cardiac syncope with signs of heart failure, and insisted on immediate hospital transport. Xiao Yan, however, strongly opposed moving the patient without knowing her medical history, fearing that any sudden movement could rupture a blood vessel and "kill both of them." During their heated argument, Bai Shu called Wang Qing's mother and learned that Wang Qing had an Aortic sinus aneurysm.
He then declared that moving her was imperative, as waiting could lead to the aneurysm rupturing at any moment. He quickly called Xu Yiran at Tongshan Hospital to prepare for the patient. Upon their arrival at Tongshan Hospital, Head Nurse Jiang Shan and Acting Director of Emergency Department Lu Ping'an were ready. Xiao Yan introduced herself to Lu Ping'an as the new Emergency Department doctor, recently returned from Lausanne University Hospital.
Bai Shu immediately questioned Xiao Yan's initial decision to wait for an ambulance at the scene. Xiao Yan defended her stance, stating that her caution was justified when unaware of the patient's critical medical history. An ultrasound confirmed that Wang Qing’s Aortic sinus aneurysm had ruptured into her right ventricle, causing heart failure.
Bai Shu used this as proof that his decision to move her was correct, but Xiao Yan, while acknowledging the outcome, reiterated that the "correctness of the result doesn't give justice to the process," and insisted that a doctor's ethical choice is to prioritize patient safety by adhering to proper procedures.
Later, as colleagues debated Xiao Yan's arrival, some expressing admiration for her beauty and background, Bai Shu remained critical, dismissively calling her a "pillow with an embroidered case" and accusing her of being a mercenary salesperson who would inflate patient costs.
Xiao Yan confronted him, clarifying that her interest in the product introduction pamphlets stemmed from her past experience with Rescue Without Borders in West Asia, where she had no access to advanced medical equipment, and she was merely trying to familiarize herself with her new hospital's state-of-the-art facilities. She reiterated her apology for the coffee incident but demanded he cease his baseless accusations. Bai Shu retorted that her actions during Wang Qing’s rescue proved she wasn't a qualified doctor.
Xiao Yan defiantly stated that her competence was not for him to decide. Xiao Yan and Tang Hua met with Wang Qing and her parents to discuss the surgical options. They explained that Wang Qing's ruptured Aortic sinus aneurysm required immediate surgery.
Two paths were presented: perform a Cesarean section first, then a heart repair, which carried the risk of exacerbating the mother's heart failure and potentially proving fatal; or perform the heart repair first, which risked the baby being stillborn due to extracorporeal blood circulation and general anesthesia. Wang Qing's parents urged her to prioritize her own life, suggesting they forgo the baby.
However, Wang Qing adamantly insisted on the Cesarean section first to save her child, explaining that her husband, Qin Shan, had died and she had no opportunity to bear another child. She declared her surgery was her decision and asked Xiao Yan to arrange it quickly. Wang Qing tearfully confided in her parents that her husband Qin Shan was a hero, and she desperately wanted to "leave a descendant for the hero." Xiao Yan proposed a bet: if Bai Shu could ride on her motorcycle for a spin without vomiting, she would step down. Bai Shu, despite calling the bet childish, accepted the challenge, donning a helmet. Xiao Yan then took him on a high-speed, dizzying ride that left him severely nauseated.
Upon their return, Xiao Yan confidently declared her victory and informed Bai Shu that she had already prepared a detailed surgical plan for Wang Qing, instructing him to study it thoroughly to ensure a flawless performance as her assistant. Bai Shu still refused to concede, arguing that the bet was irrelevant to surgical competence.
Xiao Yan passionately explained that her experience in war-torn regions of West Asia, where even basic medical equipment was a luxury, had given her the skills to perform complex surgeries under challenging conditions. She accused Bai Shu of lacking empathy, saying, "in your eyes, there is only illness, no patient." She then revealed the poignant reason behind Wang Qing’s unwavering resolve: her husband, Qin Shan, was a firefighter who had tragically sacrificed his life in a factory fire. Wang Qing was determined to save their child to preserve his legacy, a revelation that rendered Bai Shu speechless.
