After medical staff successfully resuscitated Dong Cheng, his wife and daughter, Xiao Man, created a commotion outside the operating room. Xiao Yan, Bai Shu, and Xu Yiran intervened to calm them down and explain that Dong Cheng had experienced sudden ventricular fibrillation during surgery, but was now stabilized and would be moved to the Emergency Department's Intensive Care Unit (EICU) after the operation. Shortly after, a patient named Luo You Zhi suddenly collapsed in the Emergency Department lobby.
Xiao Yan immediately initiated emergency care, diagnosing him with acute left heart failure caused by myocarditis. She administered furosemide, and his heart failure symptoms quickly improved. However, he continued to suffer from a persistent high fever, shortness of breath, and chills, despite comprehensive tests, including inflammatory markers, echocardiogram, and virology, all returning normal results. Luo You Zhi repeatedly expressed confusion about his condition. Bai Shu suspected Luo You Zhi's illness was linked to his work environment.
He went to Luo You Zhi's bedside to inquire about his job, learning that he worked at a small battery factory, primarily operating a stamping press. Earlier, Bai Shu had left a coffee for Xiao Yan. Later, Xiao Yan, reviewing the patient's chart, learned that Bai Shu had already inquired about Luo You Zhi's work situation. She messaged him a thank you, and realizing the potential connection, decided to investigate the factory herself.
Xu Yiran, noticing Bai Shu's unusual interest in Xiao Yan's patient and subsequent departure, playfully suggested that Bai Shu was "overly concerned" about Xiao Yan and deduced he was following her. Bai Shu denied it, claiming he was simply following up on the patient’s condition. Xiao Yan arrived at the factory, which was tightly shut. Her attempts to gain entry to investigate were rebuffed by the security guard.
She covertly observed workers dumping discarded nickel-cadmium rods into large bins without protective gear. Xiao Yan secretly collected a sample, but was spotted by security guards who mistook her for a journalist. They pursued her, but Bai Shu arrived just in time, rescuing her and driving her away. Xiao Yan, having twisted her ankle, couldn't ride her motorcycle, which Bai Shu had arranged to be towed back to the hospital.
Recognizing the factory's unsafe working conditions, Xiao Yan suspected Luo You Zhi's illness was caused by harmful substances, which had not yet been fully metabolized from his body, preventing his recovery. She immediately called the Administration of Work Safety to report the illegal factory. Back at the hospital, laboratory tests confirmed that Luo You Zhi was suffering from acute cadmium poisoning, which had triggered his myocarditis.
Xu Yiran confronted Bai Shu, teasing him about his supposed disinterest in Xiao Yan while secretly helping her and expressing concern for her safety. Xu Yiran directly asked if Bai Shu had developed feelings for Xiao Yan, which Bai Shu vehemently denied, citing his son and jokingly asking why he would seek out more trouble with women like Xu Yiran does.
Later, Hao Jia Jia rushed into the Emergency Department with fake blood smeared on her face, claiming she was assaulted while walking her dog and taking photos for evidence. Xiao Yan examined her and found no actual wounds. Hao Jia Jia admitted to using prop blood plasma bought online, hoping to ensure the alleged assailant's prosecution. Xiao Yan refused to falsify the diagnosis, despite Hao Jia Jia's pleas and offers of money.
Hao Jia Jia then claimed to have a concussion and demanded a CT scan. Xiao Yan informed her that a CT wouldn't typically detect a concussion, but Hao Jia Jia insisted. The scan showed no abnormalities, and Hao Jia Jia left in a huff, upset that Xiao Yan wouldn't cooperate. That evening, Hao Jia Jia posted her fabricated story online, garnering significant public outrage and sympathetic comments.
However, the next day, the man she accused also posted his version of events. He stated he had not been drinking, was walking his daughter when Hao Jia Jia's unleashed dog lunged at his child. In the ensuing argument, Hao Jia Jia insulted his daughter, saying she was worth less than the dog, leading to a physical altercation where he accidentally bumped her head, but did not beat her.
He revealed that after Hao Jia Jia's post, he had been doxxed, his house vandalized with paint, and he received threatening phone calls against his family. Tang Hua discussed the situation with Xiao Yan, lamenting the power of online mobs and the quick spread of misinformation.
Soon after, the factory boss, accompanied by his workers, stormed into the hospital, demanding 500,000 yuan in compensation from Xiao Yan for reporting his factory, which the Administration of Work Safety had since shut down. Xiao Yan publicly exposed the boss for operating without safety measures, putting his workers' lives at risk, and asked Tang Hua to call the police. The boss, in turn, accused Tongshan Hospital of being "black-hearted" and attempting to defraud his workers.
During the heated confrontation, the boss suddenly collapsed. Xiao Yan immediately examined him and diagnosed him with acute cadmium poisoning, just like Luo You Zhi. Tang Hua berated him for harming himself and others. The boss, eager to be discharged, was informed by Tang Hua that he was suspected of disturbing public order and would face police investigation after recovery.
A video of the confrontation, edited to appear unfavorable to the hospital, was posted online by an unknown bystander, leading to widespread public questioning of the EICU's effectiveness and costs. Dean Jiang Zhongjing, after reviewing the video, advised Lu Ping'an to handle the situation cautiously to avoid further complications. Tang Hua expressed her frustration, realizing the damaging power of cyberbullying. Hao Jia Jia returned to the hospital, again demanding that Xiao Yan issue a false diagnosis report. Later, a patient presented with severe right-sided lower back pain. His wife was anxious for immediate surgery based on a prior CT scan, which indicated ureteral stones. However, after a quick examination, Xiao Yan suspected he had a more serious condition, noting symptoms like tearing pain, chest tightness, and nausea, and recommended a cardiac examination. The patient's wife vehemently opposed this, accusing Xiao Yan of "over-medicalizing" for profit, and insisted on adhering to the ureteral stone diagnosis.
Bai Shu arrived and calmly pulled the wife aside, explaining the importance of Xiao Yan's judgment by sharing a past case where a patient with seemingly unrelated initial symptoms refused a heart check, only to return critically ill. He emphasized that doctors are not adversaries and that additional checks, though costing time and money, could offer peace of mind. Persuaded by Bai Shu, the wife allowed the cardiac examination.
Xiao Yan performed an electrocardiogram (ECG) and informed Bai Shu that while it appeared largely normal, it did not rule out an aortic dissection. She quickly arranged for an enhanced CT scan. The results confirmed Xiao Yan's suspicion: the patient had a type B aortic dissection with a high risk of rupture, requiring immediate surgery. The patient's wife, now understanding the severity, sincerely apologized to Xiao Yan for her earlier behavior.
Bai Shu complimented Xiao Yan's decisive judgment despite recent negative publicity, to which Xiao Yan jokingly called him "arrogant on account of his seniority."
