Head Nurse Jiang Shan presented Wang Qing's parents with the pre-surgery information, which they signed. Wang Qing, recalling her mother’s difficult pregnancy with severe pregnancy-induced hypertension and how she risked her life to give birth, assured her parents that she would be fine and aimed to be a good mother like her own. Despite their worries, Wang Qing reiterated her determination and unwavering belief in her well-being.
Meanwhile, Bai Shu, who had previously declared he would lead Wang Qing’s surgery, reviewed Xiao Yan’s proposed surgical plan. He identified what he considered to be "loopholes" and sent his detailed modification suggestions to her email, asserting that she needed someone more capable to assist her. During a confrontation, Bai Shu insisted on his professional competence, stating his plan would have no issues.
Xiao Yan, equally resolute, declared her intention to lead the surgery herself and told Bai Shu to withdraw if he maintained his opposing view. However, Director Jiang Zhongjing had already assigned Bai Shu as Xiao Yan’s assistant for the combined C-section and heart repair surgery. The surgery commenced, with Xiao Yan skillfully performing the procedure. When a small blood vessel ruptured, Bai Shu and Xiao Yan worked together seamlessly to swiftly repair it, successfully completing the operation.
Witnessing the operation, Xu Yiran praised Xiao Yan’s precise technique, but Bai Shu, ever critical, downplayed the success, claiming that if he were the lead surgeon, the vessel wouldn’t have broken. He implied that Xiao Yan’s capacity was limited, while Xiao Yan countered that handling unforeseen incidents calmly was the true measure of a doctor, not the unrealistic pursuit of an accident-free procedure.
After the successful delivery, Wang Qing awoke, relieved and emotional to see pictures of her healthy baby boy, who weighed three catty four liang (approximately 1. 7 kilograms). Her parents had named the child Qin Xiaoshan, a tribute to her late husband, Qin Shan, who died saving others in a fire. Wang Qing expressed her gratitude for her parents’ support.
Following his successful heart tumor removal, Chen Chuan Shu recovered well, leading Bai Shu to confidently assure him of a swift recovery. Chen Chuan Shu promised Bai Shu the first copy of his upcoming book. However, while Wu Xiuzhu, Chen Chuan Shu’s wife, stepped out to get water, Chen Chuan Shu reached for a pen to continue his manuscript, accidentally fell from his bed, and tragically passed away from asphyxiation.
Bai Shu rushed to rescue him, but despite immediate and prolonged resuscitation efforts for forty minutes, it was to no avail. In the aftermath, Jiang Zhongjing arranged for a third-party medical investigation, which concluded that Bai Shu’s pre-operative diagnosis, surgical execution, post-operative care, and emergency response were all accurate and compliant with medical standards, thus exonerating him from direct responsibility for Chen Chuan Shu’s death.
Despite this, Chen Yan, Chen Chuan Shu’s grandson, demanded that Bai Shu bow three times and apologize at the funeral, threatening legal action if he refused. Chen Yan believed that his grandfather’s death was a consequence of Bai Shu's insistence on surgery. Bai Shu agreed to attend the funeral and bow, but adamantly refused to apologize, asserting his professional integrity and stating that doing so would be an insult to himself and the medical profession.
He was prepared to face a lawsuit to uphold doctors' dignity. Wu Xiuzhu then arrived, expressing understanding for Chen Yan’s grief, but stating that her husband believed in fate and that his time had come. She absolved Bai Shu of any blame and encouraged him to continue his work without feeling burdened. Jiang Zhongjing, acknowledging Bai Shu as his student and apologizing on his behalf, bowed deeply to Wu Xiuzhu, expressing his profound regret for Chen Chuan Shu's passing.
Later, Xu Yiran expressed his disapproval of Bai Shu’s rigid stance, but Bai Shu remained firm, convinced that his actions were rooted in truth. In a private moment, Bai Shu reflected on Chen Chuan Shu's unfulfilled promise of his book. At the funeral, Bai Shu stood apart from the family, silently bowing in deep respect. Subsequently, Bai Shu was informed of the hospital’s decision to transfer him to the Emergency Department.
He vehemently protested, deeming it a demotion and an insult to his specialized skills as a cardiac surgeon, jokingly suggesting he might as well go to Obstetrics and Gynecology. Jiang Zhongjing, however, explained that the transfer was not a punishment but an opportunity for Bai Shu to develop into a truly empathetic doctor, one who focuses on the patient, not just a skilled technician.
He highlighted the urgent need for talent in the newly established EICU, a critical new facility that would benefit from Bai Shu's expertise. Jiang Zhongjing issued an ultimatum: succeed in the Emergency Department or leave Tongshan Hospital permanently and never call him teacher again. Bai Shu eventually grasped his mentor’s true intentions and vowed to prove his worth in the Emergency Department. Still, Acting Director Lu Ping'an voiced his anxieties to Jiang Zhongjing about Bai Shu joining the Emergency Department.
He worried that Bai Shu’s arrogant personality would cause problems for the nascent EICU, which was already facing internal opposition, and jeopardize his own chances of shedding the "acting" title. Jiang Zhongjing, however, reassured Lu Ping'an, describing Bai Shu as a promising talent who needed the challenging environment of the Emergency Department to be refined, predicting that Lu Ping'an would eventually appreciate his contributions. On his first day in the Emergency Department, Bai Shu remained disgruntled.
Xu Yiran tried to lighten the mood and introduced him to the team, including Chen Zhi and Tang Hua, who openly disliked Bai Shu's air of superiority and his pretentious habit of hand-grinding coffee. Lu Ping'an attempted to set expectations for Bai Shu’s tenure, but Bai Shu sarcastically dismissed his role as a temporary "expedient measure" by the Dean, suggesting they should avoid conflict until his inevitable return to Cardiac Surgery.
He further implied that Lu Ping'an’s emphasis on "safety first" was merely a self-serving tactic to secure a permanent directorship, which visibly angered Lu Ping'an. Shortly after, a young boy was admitted to the Emergency Department with abdominal pain, severe diarrhea, low blood pressure, and a high fever. Bai Shu quickly examined the child and, noting significant ecchymosis, confidently made a preliminary diagnosis of meningococcal meningitis.
Xiao Yan, however, questioned his rapid diagnosis, pointing out the absence of typical meningitis symptoms like headache, vomiting, or neck stiffness, and emphasizing that meningitis usually does not cause diarrhea. Bai Shu insisted on his diagnosis and ordered tests. Tests confirmed Bai Shu’s diagnosis: meningococcal meningitis complicated by Waterhouse-Friderichsen syndrome, which explained the intestinal mucosal bleeding and diarrhea.
Bai Shu, with a triumphant smirk, gloated to Xiao Yan about his accurate diagnosis, attributing her initial doubt to her not being "acclimatized" after returning from abroad. Xiao Yan calmly responded, emphasizing that no single doctor can guarantee 100% diagnostic accuracy, but collective effort minimizes errors.
Tang Hua later gossiped to Xiao Yan, suggesting Bai Shu's eagerness to diagnose the patient was a narcissistic attempt to regain attention after Xiao Yan's arrival, believing he wanted to prove his superior medical skills. Xiao Yan, unperturbed, suggested Tang Hua must have ample free time. Later, during the EICU launching ceremony, Bai Shu conspicuously turned his back to the camera during the group photo.
Jiang Zhongjing addressed the staff, stressing that the new EICU required innovative approaches and distributed Xiao Yan’s proposed operational guidelines for the Emergency and EICU, urging everyone to learn from her advanced techniques and experience from Lausanne University Hospital and her "Rescue Without Borders" work, asking her to provide further advice to the department.
Xiao Yan then pointed out a critical flaw in the hospital's traffic design, explaining that the indirect route to the emergency entrance could fatally delay treatment for patients with critical conditions like sudden cardiac arrest or major bleeding, highlighting her patient-centered approach.
