A new student has joined the Computer Science department, notable for her black, long hair, big, round eyes, and cute hat. When Tan Yueran walks by, smiling and wearing makeup, two female students find her behavior peculiar. They begin to suspect that this new, vibrant student is actually Tan Yueran. They recall that the former Tan Yueran was always somber but kind-hearted, whereas the current Tan Yueran is unsettlingly strange.
Aware of Tan Yueran's recent heart transplant, the students theorize that she is now being controlled by the heart's previous owner. This chilling idea sends shivers down Xiaoyu's and Weilan's spines, though Xiaoyu, known as "Little Yama," initially dismisses the ghost theory. Weilan decides to contact Chief Fan, Tan Yueran's treating doctor, for information. Meanwhile, Dao Ruifeng accompanies Qian Cheng for treatment at the hospital but dozes off in the patient room, prompting the doctor to angrily usher him out.
Outside, Dao Ruifeng encounters Weilan, who is there to meet Chief Fan regarding Tan Yueran's psychological state. Dao Ruifeng tries to introduce himself with an English name, "William," but Weilan playfully rebuffs him. Weilan mentions her worry about Xiaoyu, especially regarding a new man in her life, because Xiaoyu has been uncharacteristically silent about him. Weilan reveals her long-standing friendship with Xiaoyu, spanning twenty years, during which Xiaoyu transitioned from being her mother's patient to her own.
She sincerely hopes Xiaoyu lives long enough to grow old. Through surveillance footage, Jiang Baiju spots Tan Yueran, her unusual presence immediately signaling to him that something is amiss. He instructs his team to investigate her. Jiang Baiju and Qian Tu covertly enter Tan Yueran's home. Qian Tu expresses concern that their unauthorized entry is illegal, but Jiang Baiju, having obtained the address and key, assures him he’s merely there to observe.
Inside, they discover a novel titled "The Love Story of Ms. Luosifen and Mr. Coffee." The walls are covered with photographs of Fu Liang, many marked with red pen, creating an eerie atmosphere that makes Qian Tu feel like he's stepped into a serial killer movie. Elsewhere, Fu Liang visits Cai Miao’s grave, where he finds a fresh bouquet of flowers.
He reflects on their shared past and decides to bid Cai Miao a final farewell, intending to move on and start a new life in a different city. He tells Cai Miao that they never got a proper goodbye last time, and this time, he wants to do it right. Unbeknownst to him, someone silently appears behind him. Meanwhile, after their investigation, Jiang Baiju and Qian Tu realize that Tan Yueran poses a threat to Fu Liang.
They quickly gather Xiaoyu and others to locate him. Fu Liang is already bound in Tan Yueran's room. Tan Yueran reveals that her heart is Cai Miao's, stating, "Cai Miao is still alive. Cai Miao's heart still beats." She claims, "I am Cai Miao," asserting her new identity. Fu Liang, repulsed, plays along to gauge her state of mind.
He tells her he thought she was dead, which is why he planned to leave; now that he knows she's alive, he won't. He proclaims his love for Cai Miao's "soul, not her flesh." Tan Yueran, however, is consumed by her belief that he is abandoning her. She recounts how, despite her achievements, she always felt a sense of pity from others until she met Cai Miao, who helped her truly see and appreciate the world.
She cannot comprehend how Fu Liang could forsake "her" given their deep connection. As Tan Yueran produces a pair of scissors, intending to keep Fu Liang with her forever, Jiang Baiju, Xiaoyu, and Qian Tu burst in. The timely arrival of a delivery person at the door provides a momentary distraction, enabling the rescue. Fu Liang, despite the ordeal, doesn’t blame Tan Yueran.
He gently tells her that she is Tan Yueran, unique and irreplaceable, and that Cai Miao’s gift of a new heart was meant for her to live as herself, not to become Cai Miao. He vividly recalls Cai Miao's joy on the day he proposed and believes she would want them all to live full and happy lives. Fu Liang reaffirms his decision to leave the city and begin a new chapter.
Tan Yueran slowly starts to recover from her altered state, describing the experience as a long dream where a "minor role" was suddenly thrust into the "leading character's seat," leaving her disoriented by overwhelming joy. She also shares that a friend from an online forum recommended a "special medicine," which she now suspects was problematic and has given to Dr. Zhou for investigation. Xiaoyu asks for any clues, which Tan Yueran is willing to provide.
Meanwhile, Xue Wanwan repeatedly calls Dao Ruifeng, but he deliberately ignores her calls. He explains to Qian Cheng that he refers to her as "Xue Pan" (literally "Xue Plate," implying a big face and frail heart) because having known her since kindergarten, including her bedwetting habits, he lacks the "mystery" crucial for a romantic relationship.
When Weilan calls, Dao Ruifeng immediately answers and enthusiastically agrees to meet her for dinner at the hospital canteen, much to Qian Cheng’s amusement, who teases him about "courting" Jin Xiaoyu's friend. Dao Ruifeng claims he’s merely gathering "research" for a female doctor character in his novel.
Weilan expresses concern about Xiaoyu and Jiang Baiju's burgeoning relationship, asking Dao Ruifeng to look after Xiaoyu's well-being and, if things go wrong, to "speak well of her" and "get her some compensation for the break-up." Dao Ruifeng, however, thinks they might just share a "brotherly love." Weilan simply wishes for Jiang Baiju to be good to Xiaoyu. Unaware of Weilan's worries, Xiaoyu and Jiang Baiju are at home, happily making a cake. It's Xiaoyu's birthday, and despite Jiang Baiju’s centuries-long existence, he encourages her to make a wish. Without hesitation, Xiaoyu closes her eyes and wishes "to have a romantic relationship."
