However, officials like Li Shichang oppose the plan, fearing the physicians might bring the plague back to the capital and questioning the use of national funds for commoners. Zhang Ruizhong suggests a ruthless "decisive measure": abandoning Huangpi Village entirely. Xiao Chi is outraged, arguing that while he kills enemies in war, the villagers are innocent subjects.

Reminding the court of the tragedy eight years ago, Xiao Chi wins the support of Emperor Liu Chu, who orders him and Han Yunling to lead the relief mission, promising they will be honored as key ministers if they succeed. Xiao Chi is deeply concerned for Han Yunling’s safety, knowing the mission is a life-or-death gamble. However, Han Yunling is resolute, viewing the healing of the sick as her duty.

With an imperial decree granting her authority over the Imperial Medical Academy's supplies, she prepares medicine for her brother, Mo, and promises to save him once she returns. As they prepare to depart, Zhang Ruizhong sends a coffin as a "gift," mockingly wishing Xiao Chi a promotion and wealth—a cruel omen suggesting the General will not return alive. Ignoring the provocation, Xiao Chi sets off for the village.

The scene at Huangpi Village is devastating, with the death toll rising and villagers feeling abandoned by the court. The arrival of General Xiao provides a flicker of hope, and the people begin to cooperate. Physician Chen Yuan informs them that the symptoms are strikingly similar to the "Death Plague," a highly infectious disease with no known cure. Using her uncle's ancient medical notes, Han Yunling organizes a specialized treatment area to isolate the infected.

Orphaned sisters Cuizhi and Cuimiao, who lost their parents to the disease, volunteer to assist in the quarantine zone, inspiring other young men to join the effort. While Han Yunling works tirelessly, she makes a shocking discovery: the pulse of the infected Cuizhi shows clear signs of poisoning. It becomes evident that the "epidemic" is not a natural disaster but a deliberate act of human malice.

Meanwhile, a new crisis emerges as Zhang Ruizhong’s guards blockade the village under "orders from above," refusing to let anyone leave for herbs. Xiao Chi is nearly forced to use violence to break the siege until Fei Zheng arrives with an imperial decree and a shipment of supplies—including Atractylodes to fumigate the village—sent by the Eldest Princess Liu Xie. To find a cure, Han Yunling needs to test a potent but dangerous remedy.

Fei Zheng offers to return to the capital to find a death row inmate for testing, warning Xiao Chi against reckless actions. However, with time running out and the villagers dying, Han Yunling contemplates testing the medicine on herself. Xiao Chi intervenes and, in a desperate act of sacrifice, consumes the toxic blood taken from Cuizhi's fingertip to infect himself. He argues that his own body is already compromised and that Han Yunling is too important to risk.

The poison takes hold quickly. That night, Xiao Chi collapses, vomiting black blood and burning with a high fever. Han Yunling is struck with urgency, realizing that the remedy is struggling against his naturally weak constitution. Seeing that he may not survive the night, she decides to use a risky acupuncture method to save him, instructing Physician Chen Yuan to help undress the General so she can begin the treatment immediately.
