Pei Yu’s career faced a crisis when her patient, Liu Jia, died after taking her prescription. While Physician Liu mocked her and Eunuch Guo moved to have her investigated by the Palace Investigation Bureau, Pei Yu insisted her formula was safe, noting that her mentor, Physician Song, had previously vetted it.
Since the body had already been cremated to prevent the spread of the disease, she begged Li Peiyi to help her find the utensils Liu Jia had used to prove the medicine itself was not lethal. During this time, Xiao Huaijin discovered that Li Peiyi had contracted the epidemic.
Weak and fearful, she gave Xiao her final instructions through her door, asking him to look after the aging Du Zhixing and help arrange a marriage for her attendant, Wuren, with Gu Lingzhou. Distraught, Xiao sought medicine and even turned to divination for her safety, a gesture that deeply touched her. To clear Pei Yu's name and test the cure, Li Peiyi courageously volunteered to try the medicine herself, and she eventually made a full recovery.
Xiao’s investigation later revealed that Liu Jia had a pre-existing stomach condition, which he had hidden by taking stomach medicine before the trial. Furthermore, the "random" selection of Liu Jia was rigged by Physician Huang Futong, who used alum water to make the target slip reveal its writing when touched by Liu Jia's damp fingers. Despite this discovery, Huang Futong remained arrogant because the physical evidence had been destroyed.
Soon after, Physician Song was found dead, having apparently suffered a stroke and drowned in his basin of medicinal steam. Huang Futong immediately accused Pei Yu of murder, citing a recent argument between her and her mentor, though Pei Yu maintained that such bickering was a normal part of their relationship. As the young prince recovered under Pei Yu's care, Su Jieyu requested a reward for her.
Seeking to set a trap, Huang Futong praised Pei Yu before the Emperor to force her into a public oath to eradicate the epidemic within fifteen days or resign. Pei Yu overheard Huang and Physician Liu mocking her, realizing they were certain she would fail. However, Huang's arrogance turned to terror when he found a headless rat in his bed, which he interpreted as a death threat.
Although Xiao determined that a weasel had actually bitten off the rat's head, Huang was killed shortly after when a heavy millstone was triggered to fall from a roof, crushing him. Investigating the scene, Xiao and Li Peiyi realized the killer had used a rake and a rope to drop the stone from a nearby room.
While Physician Liu tried to pin the crime on Pei Yu, she remained focused on a more pressing crisis: the current cure required rare ingredients like rhino horn and pearls, which were too expensive for the palace servants. Pei Yu dedicated herself to finding an affordable alternative as the death toll among the lower ranks continued to rise.
