Knowing Zhang Dama trusted Jiang Shan, Bai Shu asked the nurse to persuade her. The X-ray revealed a moderate amount of pleural effusion. Bai Shu then suggested extracting the fluid for further examination. Zhang Dama strongly refused, fearing harm, but with Xiao Yan's agreement that further investigation was needed and Jiang Shan's encouragement, she reluctantly agreed to the thoracentesis.
However, Xiao Yan, observing Zhang Dama's frail condition and lack of symptoms, suggested against the invasive procedure, but Bai Shu remained firm in his conviction. During the thoracentesis performed by Jiang Shan, Zhang Dama suddenly experienced severe shortness of breath. Bai Shu and Xiao Yan rushed to her side and diagnosed a right pneumothorax. They immediately initiated resuscitation, stabilizing her condition.
When the pleural fluid culture results returned, they revealed cancer cells, confirming Bai Shu's insistence on the invasive procedure was correct. The next morning, Fu Mu Yao, a former patient of Xiao Yan's who had been awaiting a lung transplant after being discharged, was rushed to the Emergency Department at Tongshan Hospital after experiencing a sudden relapse while grocery shopping with her daughter, Yi Yi. Bai Shu took over her case, despite Xiao Yan's earlier treatment of her, and insisted on inserting a chest tube, overriding Xiao Yan's concerns about its invasiveness compared to a pigtail catheter.
Both Tang Hua and Lin Zhi noticed Bai Shu's noticeably different, more aggressive attitude towards Xiao Yan, but Xiao Yan dismissed their observations as baseless speculation. Xiao Yan diligently searched for a suitable lung donor for Fu Mu Yao and eventually found a match in Nanjing. Xiao Yan stayed steadfastly by Fu Mu Yao's bedside.
