In the hospital, Zhang Ying Ying was scolded by her mother, Li Chun, for speaking ill of her father, Zhang Jin Shan, in front of others, reminding her that he was still her biological father and cared for her. Ying Ying, however, remained defiant, taunting him for not trusting doctors if he was going to come to the hospital. Zhang Jin Shan became enraged, threatening to "deal with her" when he recovered.
Li Chun then urged Ying Ying to leave the room. As Ying Ying stormed off, she declared to her mother that she hated seeing "that person." Moments later, Zhang Jin Shan's condition rapidly worsened, exhibiting symptoms of kidney failure, liver failure, and respiratory failure. Bai Shu and Xu Yiran immediately initiated emergency treatment, ordering an oxygen mask, intravenous medications, arterial blood gas tests, and a chest CT scan, also noting his jaundice and suspecting liver complications.
Meanwhile, Xiao Yan contacted one of Lin Zhi Yuan's elementary school friends, whose contact information was provided by Teacher Wu Gui Fang. Xiao Yan was hoping to locate "Smoke Alley"—a place Zhi Yuan had promised to take her, but the friend couldn't provide the exact location. Bai Shu, who happened to overhear Xiao Yan's phone call, offered to take her there, explaining that his brother and sister-in-law used to live in that alley, and he visited frequently as a child.
While the alley was officially named Well Alley, an old grandmother who lived deeper inside affectionately called it Smoke Alley, a name known only to its long-term residents. Xiao Yan's excitement grew as Bai Shu mentioned the old grandmother had a grandson who went overseas, confirming Xiao Yan's suspicion that this was Lin Zhi Yuan's grandmother. They found the grandmother's courtyard gate locked, but the property appeared well-maintained.
Bai Shu inquired with neighbors and learned the house was listed for rent online. Xiao Yan, having recognized a dog that reminded her of a friend, declared her intention to rent the house, confirming to Bai Shu that she had returned to Shanghai to find a loved one. Before Xiao Yan could elaborate further, Bai Shu received an urgent call from his department and rushed back to the hospital.
Upon his return, Xu Yiran informed Bai Shu that Zhang Jin Shan’s test results ruled out uremia, suggesting poisoning instead, possibly rat poison. However, Zhang Jin Shan had undergone a full physical examination six months prior with no anomalies, and his family denied any contact with rat poison. Li Chun mentioned Zhang Jin Shan's heavy drinking and recent intake of Chinese medicine.
Bai Shu obtained the prescription and consulted Director Qin from the Chinese Medicine Department, who confirmed it contained only common ingredients incapable of causing such severe reactions. Bai Shu was at a loss to explain the rapid decline. Xiao Yan later suggested that Zhang Jin Shan’s symptoms — multi-organ failure, including kidney, liver, and progressive respiratory failure — were highly consistent with paraquat poisoning. Bai Shu's eyes lit up; paraquat, a highly fatal herbicide, had no known antidote.
He immediately questioned Zhang Jin Shan and his family about any contact with paraquat, but they all denied it. Bai Shu then instructed Chen Zhi to collect a urine sample for paraquat concentration testing. Chen Zhi brought the sample to the lab but found it missing after stepping away for a moment, leading to a scolding from Xu Yiran.
Chen Zhi went back to collect another sample from Zhang Jin Shan, who initially resisted, but Xu Yiran managed to get a new sample for analysis. While waiting for the results, Bai Shu suspected foul play, believing someone had stolen the first sample to conceal the truth. He reviewed the lab corridor's surveillance footage, which clearly showed Zhang Ying Ying taking the urine sample. With this evidence, Bai Shu immediately called the police.
The second urine test confirmed a paraquat concentration of 0. 9, far above the normal negative result. Bai Shu summoned Li Chun and Zhang Ying Ying to his office and showed them the surveillance video. After a brief denial, Zhang Ying Ying admitted to stealing the urine sample, explaining she wanted her father to remain hospitalized and not be discharged because she hated him. When pressed, she confessed to poisoning him.
Li Chun quickly interjected, confessing that she was the one who added paraquat to Zhang Jin Shan's Chinese medicine, insisting Ying Ying was not involved. Li Chun explained she was Zhang Ying Ying's stepmother. Their family business had collapsed, plunging them into debt, which turned Zhang Jin Shan into an abusive alcoholic.
She showed her old scars, recounting how he had broken her arm once and how, despite her attempts to involve the police and even initiating divorce, he would retaliate by harming her family or becoming even more violent. She explained that Ying Ying had always protected her from Zhang Jin Shan's beatings, but Zhang Jin Shan also began to beat Ying Ying, even causing her stomach to bleed once.
However, one night, after Zhang Jin Shan, in a fit of rage over leg pain, violently slammed Ying Ying's head against a wall, causing her to lose consciousness, Li Chun decided to get a bottle of paraquat from a friend and secretly added it to her husband's medicine. Ying Ying then revealed that she had seen her mother hide the paraquat bottle and realized her mother was responsible after hearing doctors suspect paraquat poisoning.
She stole the urine sample on her own initiative, hoping to keep her father sick and hospitalized, preventing him from harming them further. With the truth exposed, Zhang Ying Ying asked to see her father one last time. Bai Shu accompanied her and Li Chun to Zhang Jin Shan’s ward. Both women confessed to poisoning him. Zhang Jin Shan was furious, screaming threats and demanding the police be called. Bai Shu informed him the hospital had already called the police.
Ying Ying then declared that she was disowning him, stating that the paraquat was her way of repaying his years of abuse, and she never wanted to see him again, warning him of double retaliation if he ever bothered them. Soon after, police arrived and took Li Chun and Zhang Ying Ying in for investigation. Xu Yiran commended Bai Shu on his deduction regarding the paraquat.
Bai Shu merely commented that the law would decide their fate, and Zhang Jin Shan would now have to pay for his own care worker. Away from the hospital, Xiao Yan met with the house owner of the "Smoke Alley" courtyard. She signed a one-year rental contract and immediately arranged for cleaning services to prepare the house. Back at the Emergency Department, two car accident victims arrived.
Zhou Wen Lu, a young woman hit by the car, presented with critically low blood pressure and a high heart rate. Bai Shu quickly diagnosed her with hemorrhagic shock due to extensive chest and abdominal injuries, ordering immediate IV fluids, a bedside ultrasound, and red blood cell transfusions, indicating the need for urgent surgery. The other patient, Pan Dong, a young man who had bravely pushed Zhou Wen Lu out of the path of a second car, was unconscious.
Xiao Yan, after a CT scan, diagnosed him with a cerebral hernia, a severe brain injury, and ordered mannitol to reduce intracranial pressure, intubation, and preparations for neurosurgery. A dilemma arose as only one operating room was available. Both Xiao Yan and Bai Shu's patients required immediate surgery.
Bai Shu, learning that an appendectomy in another operating room would finish in about 30 minutes, decided to yield the current operating room to Xiao Yan, as his patient still needed a pre-surgical examination that would take approximately the same amount of time. Zhou Wen Lu's parents soon arrived, distraught and arguing. The mother blamed herself for pressure Zhou Wen Lu into a blind date, causing her daughter to storm out without eating and get into the accident.
The father retorted that he would support their daughter even if she never married, questioning the mother's methods. They then confronted the medical staff, questioning their competence and demanding immediate action, but Xiao Yan intervened, calming them and vouching for Bai Shu's expertise before he took his patient to the newly available operating room. During Zhou Wen Lu's surgery, performed by Director Lu, a complication arose with splenic bleeding. Director Lu called Bai Shu for assistance.
Bai Shu successfully located and repaired the bleeding point, and the surgery was completed. Later, Pan Dong woke up after his successful cerebral hernia surgery. He pleaded with the doctors not to inform his parents of his injury, fearing they would seize the opportunity to force him back to Canada for more unwanted blind dates.
Tang Hua remarked on the universality of generational conflicts regarding marriage, but Xiao Yan interjected, emphasizing that while differing views are common, communication and respect for children's wishes are key to resolving conflicts. Xiao Yan then approached Bai Shu, thanking him for yielding the operating room. Bai Shu downplayed it, stating it was all for the patients, but Xiao Yan noted that he was "different from how I thought of you."
